We collect the information we need to provide and manage your supports. This can include your name, contact details, NDIS number and plan details, information about your support needs and goals, and the contact details of people involved in your care.
Why we collect it
We use your information to plan and deliver your supports, to claim payment for them, to keep you safe, and to meet our obligations as a registered NDIS provider.
Who we share it with
We only share your information with your consent, or where the law requires or allows it. For example, with the NDIA or your plan manager to process payments, or with the NDIS Quality and Safeguards Commission.
How we store it
We keep your information secure and only let staff who need it see it.
Seeing or correcting your information

Forms on this website
When you send us a message or referral through this website, the details you enter are sent to our office by email through FormSubmit, a form delivery service. Please do not include detailed medical information in website forms.
